CPT 97155 is adaptive behavior treatment with protocol modification: the BCBA works face-to-face with one client to change or refine the treatment protocol, sometimes while directing an RBT in the same session. It is billed in 15-minute units.

When 97155 applies
- The BCBA is with the client and changes a target, prompt level, reinforcement plan or procedure
- The BCBA demonstrates a new or changed procedure to the RBT with the client present
- The BCBA delivers treatment directly while adjusting it based on the client's response
Overlap with 97153
If the BCBA and RBT are both in the session, payers take different approaches. Some allow 97153 and 97155 at the same time, and some pay only one. Time spent watching without changing anything may be treated as supervision instead of 97155. Check your payer's rules before scheduling overlap. See also 97153.

Frequently asked questions
- Who can bill 97155?
- A BCBA or other qualified healthcare professional the payer recognizes. Some payers also allow a BCaBA under supervision.
- Is 97155 the same as RBT supervision?
- Not always. The same visit can meet BACB supervision requirements and be billed as 97155, but only when the BCBA actually changes the protocol with the client present.
- Does the client need to be present for 97155?
- Yes. Caregiver-only meetings are billed as 97156.
