ABA CPT codes explained

ABA therapy is billed with the adaptive behavior CPT codes 97151–97158. The three most used are 97153 (direct treatment by a technician, usually an RBT), 97155 (treatment with protocol changes by the BCBA) and 97156 (caregiver guidance by the BCBA). Most are billed in 15-minute units.

How 15-minute units work

Four units make one hour. Many payers follow a midpoint rule: a unit counts once more than half of it (8 minutes or more) is delivered, but some payers round differently. Units delivered count against the authorization for that code — see tracking authorized ABA units.

A typical week

  • RBT delivers 3 hours of direct treatment, 4 days a week: 12 hours = 48 units of 97153
  • BCBA joins one session for 1 hour to adjust a protocol: 4 units of 97155
  • BCBA meets the parents for 45 minutes: 3 units of 97156

What payers usually want in the note

  • Date, start and stop times, and place of service
  • Who delivered the service, with their credential
  • The treatment-plan goals worked on and the client's response or data
  • For 97155: what was changed in the protocol and why
  • For 97156: who attended and what caregiver skills were taught
  • Signature, plus any supervisor co-sign the payer requires

How many minutes is one ABA unit?
Most adaptive behavior codes are 15-minute units, so 4 units equal 1 hour. Check your payer's rounding rule for partial units.
